Nationwide^® will discontinue provider network operations effective January 1, 2008.                                                         
The following information is provided as a reference to assist you in the resolution of outstanding claims or contact questions.
          

Nationwide Provider Network

If you are a provider contracted with Nationwide, you may access the Nationwide Provider Network page to learn more about filing claims and appeals or to view the Nationwide Provider Administration Manual.

All Other Providers

If you are not contracted with Nationwide and have questions pertaining to claims or eligibility status, please refer to the patient's member ID card effective on the date of service.
